Liability Waiver: I understand that participation in interscholastic and other voluntary after school athletics includes risk of injury that may range in severity from minor to disabling to even death. Although serious injuries are not common in supervised enrichment programs, it is impossible to eliminate the risk. I/we understand that my child’s participation in Leap PreSchool Academy after school programs is voluntary and that my child and I/we are free to choose not to participate. I/we consent to my child’s participation in any of the approved athletic activities checked above and to accompany any school team of which she/he is a member on its local or out-of-town trips. I understand that Leap PreSchool Academy, their employees, officers and agents will not be liable for personal injuries and/or property damage because of my child’s participation in such programs or activities. I/We, on behalf of myself and my minor child, agree to release, hold harmless and indemnify Leap PreSchool Academy, their employees, officers and agents, from any loss, cost, damage and/or expense of any nature, including all attorneys’ fees and costs which I or my child may have resulted, either directly or indirectly, from my child’s participation in Leap PreSchool Academy's voluntary athletic or extracurricular programs or activities. I/We give permission for our son/daughter to participate in all school activities and do forever release Leap PreSchool Academy and its teachers, staff, volunteers and agents from any and all actions, all known and unknown personal injuries or property damage of said minor arising out of said activities, and also all claims or right of action for damages which said minor has or hereafter may acquire. I also have planned to secure timely pick up of my child at the conclusion of each scheduled program event, meeting, or practice. I also understand that the Late Fee policy will apply By signing this Agreement, I/we acknowledge that we have read and understand this document and accept the risk and responsibility of participation in interscholastic or other voluntary after school athletics.
In the event of an emergency, I hereby certify that I am the parent/lawful guardian of {name-3} and grant to Leap PreSchool Academy, its employees and agents full authority to take whatever action they may consider appropriate under the circumstances involved regarding the health and safety of my child and authorize them to obtain emergency medical or dental services for my child, if necessary, at my expense.
Throughout the school year, students may be highlighted in efforts to promote LEAP PRESCHOOL ACADEMY activities and achievements. For example, students may be featured in materials to train instructors and/or increase public awareness of our enrichment programs through newspapers, radio, TV, social media, DVDs, displays, brochures, websites and other types of media. I, as the parent or guardian of {name-3}, hereby give LEAP PRESCHOOL ACADEMY and its employees, representatives, and authorized media organizations permission to print, photograph, and record my child for use in audio, video, film, or any other electronic, digital and printed media. This is with the understanding that neither LEAP PRESCHOOL ACADEMY nor its representatives will reproduce said photograph, interview, or likeness for any commercial value or receive monetary gain for use of any reproduction/broadcast of said photograph or likeness. I am also fully aware that I will not receive monetary compensation for my child’s participation. I further release and relieve LEAP PRESCHOOL ACADEMY, its Board of Trustees, employees, and other representatives from any liabilities, known or unknown, arising out of the use of this material. I certify that I have read the Media Consent and Release Liability statement and fully understand its terms and conditions. Please understand that failure to return this release form within ten (10) calendar days from the date of distribution will constitute approval of the above requests.
